Cited Napa Wine Country Sweepstakes
1.Sponsor
The Cited Napa Wine Country Sweepstakes (the “Sweepstakes”) is sponsored by CovalentCareers, Inc. (“Sponsor”).
2.Eligibility
The Sweepstakes is open only to licensed retina specialists and vitreoretinal surgeons who:
- Are legal residents of the United States;
- Are at least eighteen (18) years of age at the time of entry;
- Possess a valid National Provider Identifier (“NPI”); and
- Create a Cited account during the Sweepstakes Period.
Employees, officers, directors, contractors, and agents of Sponsor, its parent companies, subsidiaries, affiliates, advertising and promotion agencies, and members of their immediate families (spouse, parent, child, sibling, and their respective spouses), and persons living in the same household are not eligible to participate.
Void where prohibited by law.
3.Sweepstakes Period
The Sweepstakes begins at 12:00:00 a.m. Eastern Time on July 15, 2026 and ends at 11:59:59 p.m. Eastern Time on July 19, 2026 (the “Sweepstakes Period”).
Sponsor’s computer shall be the official timekeeping device for the Sweepstakes.
4.How to Enter
During the Sweepstakes Period, eligible participants must:
- Download the Cited mobile application (free to download);
- Create a Cited account;
- Provide all required registration information, including a valid NPI number; and
- Complete account registration during the Sweepstakes Period.
Upon successful account creation, participants will automatically receive one (1) entry into the Sweepstakes.
Limit one (1) entry per person.
Sponsor reserves the right to verify identity, professional licensure, NPI information, and eligibility prior to awarding the prize.
Entries that are incomplete, inaccurate, fraudulent, generated by automated means, or otherwise fail to comply with these Official Rules may be disqualified.
Alternative Method of Entry (AMOE):To enter without downloading the Cited application or creating a Cited account, during the Sweepstakes Period, hand-print your full name, email address, valid NPI number, and mailing address on a 3″ x 5″ card and mail it with sufficient postage to: Cited Napa Wine Country Sweepstakes Entry, CovalentCareers, Inc., 2251 San Diego Ave #A161, San Diego, CA 92110. Mail-in entries must be postmarked by July 19, 2026 and received by July 26, 2026. All mail-in entries will receive one (1) entry into the Sweepstakes and will be treated with equal dignity to app-based entries. The one (1) entry limit per person applies across all entry methods combined.
5.Prize
One (1) winner will receive:
One (1) $2,500 gift card to The Meritage Resort and Spa in Napa Valley, California.
Approximate Retail Value (“ARV”): $2,500.
The prize is non-transferable, non-exchangeable, and may not be redeemed for cash except where required by law.
No substitution of prize is permitted except at Sponsor’s sole discretion. Sponsor reserves the right to substitute a prize of equal or greater value if the advertised prize becomes unavailable.
Any expenses not expressly stated herein, including transportation, airfare, lodging costs exceeding the gift card value, meals, incidentals, taxes, fees, gratuities, insurance, and other expenses, are solely the responsibility of the winner.
Sponsor is not responsible for lost, stolen, damaged, expired, or misused gift cards.
6.Winner Selection
One (1) potential winner will be selected in a random drawing from all eligible entries received during the Sweepstakes Period.
The drawing will occur on or about July 26, 2026.
Odds of winning depend on the total number of eligible entries received.
7.Winner Notification
The potential winner will be notified using the email address associated with the winning entry.
The potential winner must respond within seven (7) calendar days of notification.
Sponsor may require the winner to complete and return an affidavit of eligibility, liability release, publicity release (where permitted by law), tax forms, and any other documentation reasonably requested by Sponsor.
If the potential winner:
- Cannot be contacted;
- Fails to respond within the required time period;
- Fails to provide requested documentation;
- Is determined to be ineligible; or
- Otherwise fails to comply with these Official Rules,
Sponsor may disqualify the potential winner and select an alternate winner.
8.Taxes
The winner is solely responsible for all federal, state, and local taxes associated with acceptance and use of the prize.
Sponsor may require completion of a Form W-9 and may issue an IRS Form 1099 or other applicable tax documentation as required by law.
9.Publicity Release
Except where prohibited by law, participation in the Sweepstakes constitutes the winner’s consent for Sponsor and its designees to use the winner’s name, likeness, professional credentials, city, state, photograph, voice, and statements for promotional, advertising, and publicity purposes in any media worldwide without additional compensation or notice.
10.Release of Liability
By participating, entrants agree to release, indemnify, defend, and hold harmless Sponsor and its affiliates, subsidiaries, officers, directors, employees, contractors, agents, representatives, successors, and assigns from and against any and all claims, liabilities, damages, losses, costs, or expenses arising out of or related to:
- Participation in the Sweepstakes;
- Acceptance, possession, use, misuse, or redemption of the prize;
- Travel associated with the prize; or
- Any technical, administrative, or human errors associated with the Sweepstakes.
Notwithstanding the foregoing, the release and indemnification in this Section 10 shall not apply to claims arising directly from Sponsor’s gross negligence or willful misconduct.
11.Limitations of Liability
Sponsor is not responsible for:
- Technical malfunctions or failures of any kind;
- Network, server, hardware, software, or telecommunications failures;
- Lost, late, misdirected, corrupted, incomplete, or illegible entries;
- Unauthorized intervention in any aspect of the Sweepstakes;
- Human error in administration of the Sweepstakes; or
- Any injury or damage resulting from participation in the Sweepstakes.
12.Healthcare and Employer Compliance
Participants are solely responsible for ensuring that participation in the Sweepstakes and acceptance of any prize complies with their employer’s policies, institutional policies, contractual obligations, professional responsibilities, and applicable laws and regulations.
Sponsor is not responsible for determining whether participation or prize acceptance is permissible under any such policies or requirements.
13.Travel Disclaimer
The Sweepstakes prize consists solely of the gift card described above.
Sponsor is not responsible for travel arrangements, cancellations, delays, schedule changes, weather events, resort policies, availability restrictions, or any other matters related to travel or accommodations.
All travel and accommodation arrangements are solely the responsibility of the winner.
14.General Conditions
Sponsor reserves the right, in its sole discretion, to cancel, suspend, modify, or terminate the Sweepstakes if fraud, technical failures, force majeure events, or any other factor impairs the integrity or proper functioning of the Sweepstakes.
Sponsor reserves the right to disqualify any individual who tampers with the entry process, violates these Official Rules, or acts in an unsportsmanlike or disruptive manner.
15.Privacy
Information collected in connection with this Sweepstakes, including NPI information and registration information, will be used for Sweepstakes administration, eligibility verification, account administration, and otherwise in accordance with Sponsor’s Privacy Policy available atjoincited.app/privacy.
16.Governing Law and Disputes
The Sweepstakes and these Official Rules shall be governed by and construed in accordance with the laws of the State of California, USA, without regard to conflict of law principles.
Any dispute arising out of or relating to the Sweepstakes shall be resolved exclusively in the state or federal courts located in California, and entrants consent to the jurisdiction and venue of such courts.
BINDING ARBITRATION:Except where prohibited by law, any dispute, claim, or controversy arising out of or relating to this Sweepstakes or these Official Rules that cannot be resolved informally shall be resolved by binding arbitration administered by JAMS under its Streamlined Arbitration Rules and Procedures, before a single arbitrator, in San Diego, California. The arbitrator’s decision shall be final and binding and may be entered as a judgment in any court of competent jurisdiction.
CLASS ACTION WAIVER: TO THE FULLEST EXTENT PERMITTED BY APPLICABLE LAW, YOU WAIVE ANY RIGHT TO BRING, JOIN, OR PARTICIPATE IN ANY CLASS ACTION, COLLECTIVE ACTION, OR REPRESENTATIVE PROCEEDING ARISING OUT OF OR RELATED TO THIS SWEEPSTAKES OR THESE OFFICIAL RULES.
17.Winner List
To request the winner’s name, send an email tosupport@eyesoneyecare.comwithin one (1) year following the end of the Sweepstakes Period, with the subject line “Cited Sweepstakes Winner Request.”
18.Conference and Third-Party Disclaimer
This Sweepstakes is conducted independently by CovalentCareers, Inc. and is not sponsored, endorsed, administered by, affiliated with, or otherwise associated with the American Society of Retina Specialists (ASRS), the ASRS Annual Meeting, The Meritage Resort and Spa, or any conference venue.
